Make sure to fill out the entire form, including your address, the subwoofer's model number, impedance, and size. Please try to fully describe any problems that you are having.

Only submit your request once. We reply to every request, and submitting it multiple times only causes confusion with your request, and slows our response time down. Repeated submissions of the same request will be deleted, and your entire case could be rejected.
